Streptococcus pneumoniae is an opportunistic pathogen that has two main reservoir: the nasopharynx and the upper respiratory tract. The carriage of pneumococci is highest in infants and children and has a considerable impact on bacterial transmission in the population. The objective of this study was to determine the phenotypic and genotypic diversity of S. pneumoniae and to assess the prevalence and risk factors associated with nasopharynx colonization in healthy children in Bamako, Mali. From January 1 to April 30, 2004, we performed a prospective study among 96 children aged between 2 and 24 months old enrolled in 5 health centers of Bamako. Carriage of S. pneumoniae was tested using nasopharyngeal samples (NPS) collected by aspiration. All isolates were characterized by serogroup, antimicrobial susceptibility, and drug resistance determinants (penicillin binding protein (PBP) 2, PBP3, and autolysin). For epidemiological investigation, polymerase chain reaction (PCR) was used to determine the presence of genes encoding adhesin serotypes and erythromycin resistant phenotype (ermB). Pneumococcal carriage was found in 28/96 (29%) children, 11 (39.3%) in boys and 17 (60.7%) in girls. Nasopharyngeal carriage was more frequent in children aged less than 12 months (p=0.02).
